2010 ANQ MEMBER REGISTRATION Form ANQ01	Use this form for new or recurring membership to ANQ or to advise of a personal change. This form can be used for Athletes, Officials, Coaches, Committee and Volunteers. Members must submit this form to your local ANQ affiliated club. Any fees are payable to the club
2010 ANQ MEMBERSHIP CLEARANCE REQUEST Form ANQ03	Use this form if a person had previously registered with an ANQ affiliated club in the last 5 years and is not registering with the same club as last registered.
2010 ANQ MEMBERSHIP TRANSFER FORM Form ANQ03	Use this form if a person is registered with ANQ in the current registration period and wishes to transfer to another ANQ affiliated club during that same registration period. A fee is payable to ANQ for the transfer. The joining club may also require a separate payment to cover club fees.

The 2010 ANQ Membership Forms allow users to either print the form and complete by hand or complete the form online and print it for submission to your local ANQ club.

ANQ Affiliated Club Forms

2010 MEMBER REGISTRATION SUBMISSION Form ANQ02	To be used by clubs to submit member registration forms (ANQ01) to ANQ Head Office
2010 ANQ APPLICATION FOR AFFILIATION Form ANQ04	Clubs are required to complete this application and submit all relevant details as requested on the form to be considered for affiliation with ANQ. New Committees must submit this form after their AGM but no later than March each year.
ADDITIONAL or CHANGE TO CLUB OFFICE BEARERS FORM Form ANQ05	Clubs are to advise ANQ Head Office on change or addition of office bearers during the year. This form can also be used to submit additional office bearers where there is insufficient space on the Application for Affiliation Form (ANQ04)

The 2010 ANQ Forms allow users to either print the form and complete by hand or complete the form online and print it for submission to ANQ head office.
